Not All Knee Pain Is the Same
If your knee hurts, there is no need to rush into a specific treatment. The cause and severity are determined by first checking the location and circumstances of the pain and whether there is swelling, locking, or instability, followed by a physical examination and any necessary tests.
Assuming that knee pain is osteoarthritis may cause the actual problem to be overlooked. Pain may result from worn articular cartilage, but it can also develop when the synovium lining the inside of the joint becomes inflamed or the joint capsule is irritated. The tendons, including the patellar and quadriceps tendons, the ligaments that support the knee, and the tissues surrounding the menisci are also evaluated. Discomfort originating in the hip or lower back may also be felt in the knee.
If the front of the knee hurts when walking down stairs, the patellofemoral joint and movement of the kneecap are assessed. If squatting causes a sharp pain on the inner side of the joint, the meniscus and joint line are checked by palpation. If the knee feels stiff when taking the first step after rising from a chair, it is evaluated for arthritis and its range of motion is examined. Pain that occurs only when you begin walking and then eases differs in significance from pain that worsens the longer you walk and forces you to rest. Pointing with a finger to the painful area can also help during the examination.
The timing of swelling should also be recorded. If the knee swelled rapidly within a few hours after an injury, possible damage or bleeding inside the joint is evaluated. You should also tell your healthcare provider if the knee catches when bending or straightening, locks and cannot fully straighten, or feels as though it may give way when you change direction. How many minutes the joint remains stiff in the morning and whether the pain is severe enough to wake you from sleep can also help narrow down the cause.
The initial evaluation of knee pain begins with taking a medical history and performing a physical examination (an assessment in which a healthcare provider uses their hands and specific movements to check the condition of the joint) to locate the painful area and identify warning signs.(Bunt et al., 2018) Different conditions can sometimes cause similar symptoms. This is why it is difficult to make a diagnosis or select a specific injection based on only a few symptoms.
How to Differentiate Osteoarthritis, Meniscal Injury, Patellofemoral Pain, and Ligament Injury
If knee pain begins gradually after middle age without any particular trauma and worsens after prolonged walking or standing, osteoarthritis should be considered. Morning knee stiffness that improves within a relatively short time after movement is another clue. Age and pain alone do not confirm arthritis. Considering age, pain location, how the symptoms developed, any history of trauma, and mechanical symptoms such as locking helps narrow the differential diagnosis.(Bunt et al., 2018)
If the knee suddenly swells after an injury and it is difficult to bear weight, or if the joint feels unstable when changing direction, the anterior cruciate ligament and collateral ligaments should be assessed for injury. The direction of the impact, any sound heard at the time, and whether the patient was able to walk afterward also inform the examination. Ligament testing involves moving the shinbone forward and backward or applying inward and outward forces to the knee to assess instability and pain.
A meniscal injury should be suspected if the knee catches and cannot fully straighten or if twisting causes sharp pain along the inner joint line. The meniscus is tissue between the thighbone and shinbone that distributes impact. It may tear because of trauma, or degenerative changes may develop in tissue weakened with age. Even if imaging shows damage, whether it matches the current symptoms must be assessed separately.
If the front of the knee hurts when going up or down stairs or standing after sitting for a long time, patellofemoral pain syndrome (pain where the kneecap meets the thighbone) should be considered. Pain is also often worse when squatting, running, or walking downhill. Because kneecap movement, hip and thigh strength, recent activity levels, and lower-limb alignment, including the feet, can contribute to symptoms, these factors should be evaluated together.
When symptoms persist for more than three months or recur after middle age, degenerative osteoarthritis should be considered a common cause. However, age and symptom duration alone do not determine the diagnosis; tendon disorders, meniscal injury, inflammatory arthritis, and a history of trauma should also be considered in the differential diagnosis. Other structures in and around the joint that may cause pain should also be evaluated.(Vanneste et al., 2024) If a knee examination does not fully explain the current symptoms, or if hip or lower back pain occurs together with thigh numbness, the hip and lower back should also be examined. This is because pain felt in the knee may be referred pain originating elsewhere.
Each symptom is not a diagnosis in itself. It is a clue for deciding which examination to perform first and which area to evaluate with imaging.
Each Test Answers a Different Question
Having more tests does not necessarily improve diagnostic accuracy. After narrowing down the suspected structures and warning signs through a medical history and physical examination, the clinician selects the imaging study that can answer the remaining questions. The same imaging test is not used for every case of knee pain; the choice depends on the needs identified from the patient’s history and examination.(Bunt et al., 2018)
X-rays show bone shape and alignment, joint space, and osteophytes (bone growths along the edges of a joint). When degenerative changes are suspected, weight-bearing images taken while standing are useful. Because the knee bears weight in a way similar to when walking or standing, these images may reveal joint-space narrowing that was less apparent on images taken while lying down. To assess the patellofemoral joint, additional images are taken to show the surface along which the kneecap moves.
Musculoskeletal ultrasound provides real-time views of structures close to the surface, such as tendons, ligaments, fluid around the joint, and bursae. The clinician may move the knee or press on a specific area to determine whether the painful spot corresponds to a structure visible on the screen. If an ultrasound-guided injection is needed, the clinician adjusts the needle position while viewing the needle tip and target area on the screen. Some areas, such as the inside of the bone or the entire meniscus, can be difficult to evaluate adequately with ultrasound alone.
MRI provides detailed images of structures inside the joint, including the menisci, cruciate ligaments, cartilage, and the inside of the bone. It is selected when the examination suggests an internal injury or when basic imaging alone cannot adequately explain the cause. Because degenerative changes may appear on imaging even without symptoms, treatment is not determined by a single statement in the MRI report alone.
After an acute injury caused by a fall or impact, X-rays are used as the initial imaging test. Even when no fracture is visible, a follow-up MRI may be considered if an injury inside the joint, such as damage to a ligament or meniscus, is suspected.(Tuite et al., 2020) The appropriate test depends on the question that needs to be answered at the time. The first step is to determine whether the evaluation should focus on the bones and joint space, tendons in motion, or an injury inside the joint.
Five Things to Check at a Hospital and How to Design a Stepwise Treatment Plan
When choosing a hospital, focus less on the names of its equipment and more on whether it clearly explains the entire process, from diagnosis through reassessment. Check whether the medical team distinguishes when weight-bearing X-rays are needed and directly examines the joints and muscle strength through a physical examination. Also confirm whether they explain the criteria for moving to the next step before repeating the same injection. You should also determine whether the care plan includes criteria for considering surgery, a referral pathway, and muscle strength and weight management.
Treatment should be tailored to the diagnosis and the patient’s current functional status. When swelling and inflammation make even walking difficult, treatment to control pain and restore everyday mobility is considered first. After reviewing the diagnosis, symptom severity, and response to previous treatments, ultrasound guidance may be considered if an injection is indicated. When symptoms are suspected to originate from nerves in the lower back rather than the knee itself, a neurological examination is performed first to check for radiculopathy. If medication and exercise therapy do not control the symptoms, a C-arm-guided nerve block is performed after explaining its purpose and risks. Medication is administered while real-time fluoroscopy confirms the locations of the nerve and joint, and the procedure takes 5–10 minutes, allowing the patient to return home the same day. Patients typically undergo one to three sessions at intervals of one to two weeks, with their progress monitored. These two procedures target different areas and have different indications. Insurance coverage also varies depending on the medication used and the diagnosis.
The recovery phase begins after injections reduce the pain. If reduced joint range of motion or weakness in the thigh and hip muscles is left untreated, the load on the knee increases again. One-on-one rehabilitation combines joint mobilization, muscle energy techniques, and relaxation therapy to restore movement and muscle function. A therapist manually adjusts the intensity, with each session lasting 30–40 minutes, two to three times per week for 4–12 weeks. The number and duration of sessions are adjusted according to the extent of the injury, the pace of recovery, and the load placed on the knee in daily life.
If a tendon problem such as patellar tendinitis has been confirmed and pain persists despite activity modification and basic treatment, extracorporeal shock wave therapy is performed. Shock waves are delivered to the tendon at the painful site to provide mechanical stimulation to the tissue. Focused shock waves are used for deep calcification and adhesions, while radial shock waves are used for widespread fascial tension. The method is selected after ultrasound and physical examination identify the target tissue.
If pain and functional limitations remain despite these steps, regenerative treatment is considered. DNA injections contain PDRN (polydeoxyribonucleotide) and are prescription medications approved by the Ministry of Food and Drug Safety; their indications include knee osteoarthritis and meniscal injuries. They are injected around the injured area, usually in three to five sessions at intervals of one to two weeks.
PRP is prepared by centrifuging the patient’s blood to obtain platelet-rich plasma, which is then injected under ultrasound guidance. It passed the Korean National Evidence-based Healthcare Collaborating Agency’s new health technology assessment in December 2024, and its recognized indication is KL grade 2–3 knee osteoarthritis that has shown an inadequate response to medication or steroid or hyaluronic acid injections for at least one year. Surgery for degenerative meniscal lesions is not determined by MRI findings alone. For degenerative lesions in middle-aged patients, evidence concerning arthroscopic partial meniscectomy and exercise therapy is reviewed together, while symptoms, functional limitations, and the response to conservative treatment are also considered in the decision.(Rotini et al., 2023) If the knee continues to lock, a major structural injury makes it difficult to maintain function, or the condition is beyond the scope of nonsurgical treatment, the patient is evaluated to determine whether the severity warrants surgery. A medical institution that does not perform surgery explains the basis for its assessment and refers the patient to a higher-level medical institution that provides surgical care.
A treatment plan that only temporarily reduces pain is not enough. Reassessing in sequence whether the affected tissue has stabilized, joint movement is returning, and muscle strength has recovered enough to withstand everyday loads helps clarify the next step.
Signs That Should Not Be Ignored or Left Unevaluated
If the knee swells rapidly and it is difficult to bear weight immediately after a fall or twisting injury, evaluation should not be delayed. If the joint becomes red and warm along with a fever, a healthcare professional should assess the possibility of septic arthritis. A locked knee that cannot fully straighten is also a sign that the internal structures of the joint may be blocking movement and should be evaluated.
Persistent nighttime pain even at rest, pain that progressively worsens, or difficulty walking even a few steps also requires investigation of the cause. Severe trauma, possible infection, joint locking, and difficulty bearing weight are clues that indicate the need for prompt evaluation.(Bunt et al., 2018)
After treatment begins, both pain scores and knee function are assessed. Changes in knee circumference and swelling, the range of flexion and extension, limping, the ability to go up and down stairs, and thigh muscle strength are monitored. If function does not improve within the specified period or new locking or swelling develops, the diagnosis and treatment plan should be reviewed.
Preventing recurrence includes strength exercises tailored to the current diagnosis and physical condition. Rather than repeatedly strengthening only the muscles at the front of the thigh, hip strength, balance, and ankle mobility should also be addressed as needed. Patients who need to lose weight should also develop a plan to reduce the repetitive load placed on the knee. Exercise and weight loss when needed are components of nonsurgical management for knee osteoarthritis.(Brophy et al., 2022) The type and intensity of exercise and the recovery period vary depending on the condition of the joint and any underlying medical conditions.
