Why Would My Wrist Cause Numb Hands?
Have you ever been woken up in the middle of the night by numbness in your hands? Most people assume it is a circulation issue — but carpal tunnel syndrome, compression of the median nerve at the wrist, should be the first condition to rule out.
On the palm side of the wrist, the carpal bones and the transverse carpal ligament form the carpal tunnel — a narrow passage at the front of the wrist. The tendons that bend the fingers and the median nerve all pass through this tunnel together. When the tunnel narrows or the tissue around the tendons swells, the nerve gets compressed. Carpal tunnel syndrome is an entrapment neuropathy (a condition where a nerve is pinched in a tight space) caused by the median nerve becoming trapped inside the carpal tunnel. (Harinesan et al., 2024)
Because the median nerve is compressed, numbness follows the path that nerve travels. It tends to concentrate in the thumb, index finger, and middle finger; the thumb-side half of the ring finger may also go numb. (Karjalainen et al., 2023) The little finger is supplied by the ulnar nerve. If that finger is clearly numb as well, the nerve at the inner elbow or in the neck may need to be examined too.
Early on, symptoms may feel like tingling or burning at the fingertips, or simply a loss of sensation. The pins-and-needles feeling can wake you at night, yet disappear completely during the day — making it easy to dismiss. As compression continues, the hand may go numb while gripping a steering wheel or holding a phone. Fine movements like fastening buttons or picking up coins can start to feel awkward.
Carpal tunnel syndrome is particularly common in middle-aged women. Repetitive wrist flexion or forceful gripping raises the risk depending on the type and duration of exposure. It is worth checking for contributing factors such as pregnancy-related swelling, diabetes, thyroid disease, or rheumatoid arthritis — conditions that can affect the volume of tissue inside the carpal tunnel or the health of the nerve itself.
When symptoms have been present only a short time and both hand sensation and thumb strength are still intact, a nighttime wrist splint and activity modification are the right starting point. When numbness persists into the day or fingertip sensation has begun to fade, nerve conduction studies and ultrasound can quantify the degree of compression and guide decisions about injections or surgery.
Why Does the Numbness Strike Only at Night?
Many people describe being fine during the day, then being jolted awake at two or three in the morning by numb hands. During sleep, the wrist naturally bends toward the palm without you realizing it. That position raises pressure inside the carpal tunnel and squeezes the median nerve even harder. Wrist posture and the resulting changes in carpal tunnel pressure are the main mechanism behind nighttime symptoms. (Padua et al., 2023)
At night, the hand is almost completely still, so wrist flexion and shifts in tissue fluid push carpal tunnel pressure up without anything to counteract them. Even slight swelling in the tendon sheaths or surrounding tissue can meaningfully raise pressure in a space that has very little room to spare. Days of heavy hand use or any swelling in the hand tend to make nighttime symptoms worse.
When the hand goes numb, the instinct is to hang it down and shake it out. Changing wrist position and moving the fingers and tendons temporarily lowers pressure inside the tunnel, which brings brief relief — but only brief. Once the underlying source of compression remains, falling back asleep leads to the same symptoms all over again.
Median nerve compression causes pain, numbness, and abnormal sensation, and these symptoms worsen at night. (Harinesan et al., 2024) If you are waking more often and shaking your hand for longer before the feeling returns, it is time to reassess how far the condition has progressed. Tracking how many nights a week you wake up, which fingers go numb first, and how many minutes it takes for the numbness to settle after shaking can help determine whether a splint alone is sufficient or whether nerve studies are needed.
What Tests Confirm Carpal Tunnel Syndrome?
Before ordering tests, the clinician will ask about the distribution of numbness — whether it stays in the thumb through middle finger, which side of the ring finger is affected, and whether it reaches the little finger. How many times a night you wake up, whether shaking the hand helps, and any daytime sensory loss are also important. When the pattern differs from the typical picture or symptoms are severe, electrodiagnostic testing (nerve conduction studies and electromyography) can confirm the diagnosis and grade the severity. The Phalen test holds the wrists in flexion to see whether it reproduces the familiar numbness. The Tinel sign is checked by tapping lightly over the median nerve at the carpal tunnel; a tingling sensation that shoots to the fingertips is a positive result. Reproducing the usual symptoms supports the diagnosis, but a negative test does not rule carpal tunnel syndrome out. Conversely, even if numbness is reproduced, the distribution, muscle strength findings, and other test results must all fit together — if they do not, another cause needs to be considered.
When discomfort persists or thumb strength declines, nerve conduction studies measure how much the electrical signal slows across the wrist segment. This quantifies conduction abnormality and functional severity. Mild slowing calls for a nighttime splint and activity changes first. A pronounced drop in signal speed — or evidence that the motor nerve fibers are also affected — moves surgery evaluation forward.
Ultrasound lets the clinician see the median nerve directly inside the carpal tunnel on screen. It shows whether the nerve is swollen just proximal to (above) the point of compression, whether the tendon sheath has thickened, and whether a cyst or other structure is occupying space in the tunnel. When an injection is planned, ultrasound guidance lets the clinician watch both the nerve and the needle tip in real time to position the medication accurately. Symptoms, electrodiagnostic findings, and imaging are reviewed together to assess the degree of compression and the surrounding anatomy. (Padua et al., 2023)
When numbness reaches the little finger, or when turning and tilting the neck changes the sensation in the arm, wrist examination alone is not enough. Neck-to-shoulder-to-arm pain, sensory changes above the elbow, or arm weakness alongside hand numbness all warrant examination of the cervical (neck) nerves. Simultaneous nerve compression at both the wrist and the neck is possible. If the numbness seems to originate in the neck, distinguishing it from arm numbness stemming from a cervical disc herniation comes first, and only then is a decision made about whether to use a wrist splint, treat the neck, or both.
What Does Treatment Look Like, Step by Step?
When symptoms occur only at night, come and go, and both sensation and thumb strength are preserved, a nighttime wrist splint is the first step. Holding the wrist in a neutral position — neither bent up nor bent down — limits the rise in carpal tunnel pressure during sleep. Clinical guidelines and systematic reviews consistently list splinting as a primary non-surgical option for early carpal tunnel syndrome. (Karjalainen et al., 2023)
The splint should be worn consistently through the night. During the day, time spent holding the phone with a flexed wrist or gripping tools forcefully should be reduced. For people who sit at a desk for long hours, addressing neck and shoulder posture can also help reduce the load on the wrist. When inflammation and pain accompany the numbness, a short course of non-steroidal anti-inflammatory drugs may be used after reviewing existing conditions and current medications. If four to six weeks of splinting has not stopped the nightly awakenings or the daytime numbness, sensation, muscle strength, and nerve conduction results are reassessed.
For mild to moderate carpal tunnel syndrome where numbness and sleep disruption continue, a local corticosteroid injection into the carpal tunnel is the next option. Ultrasound guidance allows the clinician to distinguish the median nerve, tendons, and blood vessels, and to watch the needle tip throughout the procedure as medication is delivered into the tunnel. Reducing swelling in the tendon sheath and surrounding tissue lowers the pressure on the nerve. A meta-analysis of randomized controlled trials compared symptom outcomes and safety between ultrasound-guided injection and landmark-guided (anatomy-based) injection. After the injection, rehabilitation focused on adjusting wrist habits and movement patterns runs alongside recovery. A specialist evaluates the patient's condition and introduces tendon-gliding exercises and median nerve gliding exercises — movements designed to help the nerve slide freely between surrounding tissues — within a range that does not provoke symptoms. The goal is not simply a period of reduced numbness but a lasting change to the daily movements that repeatedly load the nerve.
When symptoms overlap between the wrist and the neck, treatment is directed based on where the examination and imaging point. If arm pain that radiates all the way down changes with neck movement, a cervical nerve block may be considered alongside neurological examination, imaging findings, and the response to conservative care. If evidence of median nerve compression at the wrist is also present, the nighttime splint and wrist treatment continue in parallel. Identifying which level is responsible is essential to avoid leftover symptoms after treating only one site.
When nighttime waking and daytime sensory loss persist despite splinting, activity modification, medication, and injection — or when nerve damage is clear on testing — carpal tunnel decompression surgery is discussed according to severity. The procedure opens the transverse carpal ligament to relieve pressure on the median nerve. Weak Thumb and Fading Sensation
When fingertip sensation is persistently reduced throughout the day — not just at night — and it becomes difficult to tell hot from cold objects, nerve compression has progressed. Reduced sensation in the hand means hot water and cooking utensils should not be handled bare-handed, and the skin should be checked regularly for burns that may have gone unnoticed. Numbness that remains after shaking the hand, or sensation that takes longer and longer to return, carries the same message. Rather than waiting with the splint alone, nerve conduction studies are needed to determine the extent of damage.
Check whether the thenar eminence (the fleshy pad at the base of the thumb) looks thinner than the opposite hand. Difficulty opening jar lids, trouble gripping keys or fastening buttons, and frequently dropping cups or chopsticks all suggest motor nerve involvement. Persistent sensory loss, declining grip and pinch strength, and thenar muscle wasting point to advanced median nerve compression. (Wipperman et al., 2024)
At this stage, surgery warrants serious consideration. When nerve conduction studies show markedly slowed or absent sensory signals, or when motor signals to the muscles are also weak, the need for decompression surgery is evaluated. Opening the transverse carpal ligament reduces pressure inside the carpal tunnel. If symptoms briefly improve after a splint or injection but muscle strength continues to fall, it is no longer reasonable to continue existing treatment based on symptom reduction alone.
The longer compression has been present, the slower nerve recovery tends to be even after the tunnel is widened. Some degree of sensory loss or reduced thumb strength may persist, so the timing of diagnosis and the severity of compression at that point both influence the long-term outlook. (Padua et al., 2023) Not everyone who develops nighttime numbness needs surgery straight away. When sensation and strength are intact and testing shows mild involvement, starting with a splint and activity modification — and monitoring progress — is the appropriate approach.
When discussing whether surgery is right for a given patient, the following factors all matter: when symptoms began, how many nights a week sleep is disrupted, which fingers go numb, any daytime sensory loss, changes in thumb strength, and any underlying medical conditions. How long splinting, medication, and injections were used, and how much symptom relief they provided, also inform the decision. Bringing a summary of these points to the appointment makes it easier to determine the scope of testing and the appropriate next treatment step.
Repeated nighttime waking, persistent daytime sensory loss, or weakening thumb strength all call for an assessment of how far nerve damage has progressed.
